HOME VISITATION

AASCSC is a non-profit organization, based in Orange County, CA (our center is in Santa Ana), that serves all age groups and all members of our community. We believe in an inter-generational approach to building a stronger, healthier community through direct services, advocacy, and capacity building. At the heart of these efforts is the imperative to ensure health equity within our community.

Home Visitation Program (HVP) supports positive health development and well-being outcomes for pregnant and parenting people, families and infants born into poverty, with the goal of expanding their future educational, economic, and financial capability opportunities, and improving the likelihood that they will exit poverty.

Home visiting is an evidence-based, voluntary program that pairs new parents with a nurse or trained professional who makes regular visits to the participant’s home to provide guidance, coaching and access to health and social services. 

AASCSC’s HVP adopts Parents as Teachers® (PAT), which is an evidence-based home visiting model and provides the following services to CalWORKs families with children under 5 years of age:

  • Health & Mental Health Screening and Assessment For Parents/Caregivers
  • Child Development Assessment and Family Review 
  • Early Childhood Education and Parenting Support
